How to Apply for Assistance
We have provided a checklist [PDF] for managing your application. Then, complete the Florida BRAIVE Fund application and attach copies of required documentation.  The documents required include:
- BRAIVE Fund Application [PDF]
- Servicemember’s Active Duty Orders
- DD Form 214 (Report of Separation), if applicable
- Applicant’s Summary of Need statement
Submit your completed application packet to the Florida National Guard Foundation, 82 Marine Street, St. Augustine, FL 32084 or via fax or email to the number or email address below. And finally, please call and confirm that the Florida National Guard Foundation office has received your application.
